Scuba Forms must be filled out under the following guidelines:
The first line on the liability release should be for “Scuba Discovery.”
The instructor can be left blank.
The facility is “Heartland Scuba Center” in “Lincoln, NE.”
The agency (at the bottom) should be “SDI.”
All blanks are to be initialed (no check marks or X’s allowed) and the form should be witnessed.
The medical form requires a “YES” or “NO” answer to each question
***The doctor’s signature on the Physician Signoff Form is only required if there is a “YES” answer to one of the questions.
